Safety Award Application

The North American Die Casting Association is pleased to recognize and present a Special Safety Award to those Corporate Members that have maintained a safety record during 2011 equal to or less than 7.0, the national average for all manufacturing.

All Corporate Members in good standing are encouraged to apply. The Safety Awards will be presented at NADCA's Die Casting Industry Gala on Tuesday, October 9, as part of NADCA's Die Casting Congress & Exhibition. Winners will also be featured in the May issue of DCE magazine and April issue of LINKS.

The following information should be available from your OSHA form 300A (Summary of Work-Related Injuries and Illnesses). Companies with more than 10 employees are required to post OSHA’s Form 300A  from February 1 to April 30 each year. Smaller companies should consult insurance or other records.

If during this time period, any fatalities occurred, your company is not eligible for a safety award.


b) Number of lost workday cases (column H)*

*A lost workday case is as any injury or illness incident that involves days away from work.
